I am really quite pleased with how the face blended into the hand overall. It was blended in with levels, soft erasing, 50% cloning, and healing. For those keeping score, the face, hand, and background were each a picture. The background was added in with a layer mask in overlay mode. The blood was just a basic blood splatter brush I got from the resources area. Levels were all brought down, with some of the creases in the hand enhanced with the burn tool. The eyes were filled in with the clone tool (they were mostly already rolled up).

*ADDITION*
It has been brought up a couple times now that this picture reminds people of Left Hand from Vampire Hunter D. While that was not a consideration in my planning and design, I have to admit, it is a striking resemblance and I can see where people get that idea. But, that isn't actually what it is, at least, not technically or something...
